Industrial Incidents Caused by Emergency Response Failure

Emergency response failures investigated by the CSB include inadequate emergency plans, failure to evacuate personnel from the hazard zone, absence of trained emergency responders, and decisions during the emergency that escalated rather than mitigated the incident. These cases document the gap between written emergency plans and actual response capability.

40 investigations
Industries: Chemical Manufacturing, Petroleum Refining, Transportation
Years: 1998–2024
