Industrial Incidents Caused by Maintenance Error¶
Maintenance errors — incorrect reassembly, use of wrong materials, inadequate isolation before work begins, or failures to restore equipment to normal operating condition — have been the direct initiating cause or a key contributing factor in numerous CSB investigations. These cases highlight the critical importance of permit-to-work systems, post-maintenance testing, and quality verification steps.
