Industrial Incidents Caused by Alarm Management Failure

Alarm management failures include alarm floods that buried critical signals, nuisance alarms that trained operators to silence without investigating, and control room configurations that made it impossible to identify the most urgent alert. The CSB has cited deficient alarm management in major incidents including the Texas Petrochemicals explosion and multiple refinery incidents, where operators were overwhelmed with alarms during the critical window before loss of containment.
